診療レポート
それは在宅医療を行っている証でもあります。
在宅訪問診療に行くと、わたしたちはその場で当日の診療内容を記載し、診療レポートとして、持参しているプリンターで印刷し、クリニックから提供している「わたしのファイル」にレポートをファイルしてお家を出ます。
本日の診療を、患者さんが読み返せる、不在だった家族があとで確認できる、関わる職種の方が情報共有できるように。そして、訪問も重なってくると、溜まったレポートは日記のようになる、いつかご家族が振り返って読めるように。
在宅の患者さんは、高齢で認知機能が低下しているひとも多くいます。ご家族と一緒に何度も読み返していただく方もいます。病状が変わりやすい心不全を在宅医療でおこなうために、診療レポートはタイムリーである必要があります。
同席できない家族に見せたい。
家族が介護を頑張った記録でもあります。
そして、私たちが、その人の人生の最後の一部かもしれませんが、一緒にいた証でもあります。
こんな思いで、少し時間をいただきますが、私たちは綴っています。
Medical Care Record
It is a proof that we provide home healthcare.
Every visit we make to the patient's home, we memoeverything we did that day and keep it as medical care record. We bring a printer to the patient's home so that we can print it out and file it into a folder we call "My File."
It allows the patient to read over what service they received on that day, lets the family members who were absent when we visited their house to check,and helps every medical staff to share necessary information. As we make more visits to the patient's home, the record piles up and start to works as adiary. Many patients who need home healthcare are old and sometimes with deteriorated mental state.There are many patients who read over the report again and again with his or her family members. In order to provide the better home healthcare for heart failure patients, this medical care records need to beupdated and timely in manner.
We want the family member to know.
It is a record for family members of what they went through supporting the patient.
Also, it is a proof for us that we were with the patient even though it may only be a fraction of their life.
That is why we take time and put our heart to make the medical care record.
