癌と心不全
２０１３年アメリカ心臓病学会での心不全ガイドラインが発表された。
２０１３ ACCF/AHA Guideline for the Management of Heart Failure
この新しいガイドラインのscopeのひとつとして
stage D（難治性心不全）に対するPalliative Care (緩和ケア）
が新しい領域として取り上げられた。
まさに、私たちのクリニックが、いま力をいれている
難治性心不全に対する治療・ケアの領域が、
世界の循環器医療のトピックスとして、注目されはじめている。
Palliative Care for Patients With HF
In newly diagnosed cancer patients, palliative care interventions delivered early have had a positive impact on survival and HRQOL. This approach may also be relevant for HF. Evaluation for cardiac transplantation or MCS in experienced centers should include formal palliative care consultation, which can improve advanced care planning and enhance the overall quality of decision making and integrated care for these patients, regardless of the advanced HF therapy selected
心不全における緩和ケア
新しく診断された癌患者のように、心不全患者においても、早い段階で緩和ケアの概念を取り入れることで、生命予後や生活の質を改善させる可能性をもつかもしれない。また本邦においてまだ一般的とは言えない、末期心不全患者における、植え込み型人工心臓や心臓移植の適応評価においても、その治療選択いかんにかかわらず、「緩和ケア」の考えを導入することの必要性を強調している。
末期心不全患者は症状緩和のため、早い段階から、多角的に、そして多職種で、
より良い生活の質の維持のため、患者、家族をケアすることは必要である。
しかしながら、癌で通常使われる「緩和ケア」と同義語で
心不全に当てはめることはできない。
心不全は癌と病気そのものが違う。
心不全は症候群であり、その症状、基礎疾患、病期は多岐にわたり、それらは変化もする。
たとえば、病状の進行により、そのステージが悪くなったかと思うと、
さらに適切な治療を行うことにより、重症度ステージの改善を認める。
「心不全だから進行性に死んでしまう、心不全＝死」
という病気ではないからだ。
もちろん、癌でもその種類や病期により同様のことが言えるが、
一般的な心不全と癌の違いはそこにある。
また当クリニックで前向きに行っている、末期心不全の在宅ケア、について
在宅でお看取りをする医療
と勘違いされるが、私たちはそのような気持ちで医療を行っていない。
末期心不全の在宅ケア≠ 在宅での看取り医療
末期心不全の在宅ケア＝生活の質の維持または改善を行うための積極的な医療
である。
どんな心不全患者でも、
○ まず従来の確立された心不全治療をきちんと行い
○ 包括的にその人にあった付加的治療やケアを更に行い
○ 経過において、様々な増悪要因（下図）をしっかりと抑え
○ そして、生活の基盤である在宅での積極的な治療・ケアを行うことにより、
病状の悪化を未然に防ぐことができれば
一般の人たちと同様に、生きていける可能性をもつ。
だから、わたしたちは、心不全患者さんが、より元気に、楽しく生活をしてもらうため
希望がある限り、とことんまで、治療・ケアを行っていく。
Cancer and Heart Failure
American Heart Association announced 2013 ACCF/AHA Guideline for the management of Heart Failure. One of the scopes of this new guideline focused on "Palliative Care" for Stage D (Advanced Heart Failure) as a new topic of treatment.
Our clinic has been focusing on palliative care for advanced heart failure and we are excited that this topic is starting to be focused in cardiovascular medicine in the world.

It is very critical not only to take care of the heart failure patients at terminal stage, but also their family members to maintain better quality of life from the early stage with all the support they need.
However, we need to be careful with the term "Palliative Care" for Heart Failure due to the differences in the nature of the disease. Heart Failure and Cancer is completely different disease.
Heart Failure is a syndrome that symptoms, underlying disease, and stages differ and change over the course of time. For instance, severity of the disease could be altered by adequate treatment even if the disease progresses.
Heart Failure does not mean it is a progressive disease that leads to death
Of course, this statement would apply to other diseases as well as cancer.
But there is a line between these two.
Some sees our homecare for terminal heart failure as just being on the patient's side at his/her last moment of life at home, but the attitude and motivation of our members are not superficial.

Homecare for terminal heart failure patients=Maintaining and/or improving the quality of life with proactive care and treatment.
For any heart failure patients,
we will
・provide evidence-proven standard treatment first.
・provide any adjunctive therapy
・eliminate all the mal-factors such as shown in the diagram
・Provide treatment and care at their home, and if exacerbation of the disease could be prevented, these patients can live just like any one of us.
This is our direction and we will continue providing treatment and care for heart failure patients so that they can have better quality of life and enjoy their lives as long as there is a hope and need for us.
