シームレスな医療とは
神奈川県病院学会で『心不全医療』について、お話しをさせていただく機会がありました
学会テーマは、『地域医療連携の推進-シームレスな環境を求めて』
シームレスな医療とはどんな医療なのでしょうか？
医療者が、
『外来』『入院』『在宅』といった医療環境を作り上げることだけでしょうか？
患者さんに合せて環境を決めることがシームレスな医療なのでしょうか？
ふと、米国ボストン、Harvard University / Brigham And Women's Hospital、
重症心不全の世界的なオピニオンリーダーである
Lynne Warner Stevenson先生の言葉が思い浮びました。

患者さんには、
これまでの人生があり、
今があり、
これからの人生があります。
誰もが、誕生し、成長し、老衰し、死を迎えます。
そこには、喜びがあり、怒りがあり、哀しみがあり、楽しみがあります。
患者さんにとって、人生はシームレスであり、日々がつながっております。
患者さんが、
これまでどんな人生を歩んできたか、
今どのような生活をしているのか、
これから何をしたいのかを知り、
それに私たちが、寄り添いながら、様々な医療形態の手段をもち、
患者さん自身の人生の環境づくりのサポートをする。
それが本当の意味でのシームレスな医療であると考えます。
今私たちのクリニックでは、そのようなシームレスな環境づくりに取り組んでおります。
90代男性Aさん、クリニックの外来へ都内総合病院からの紹介状をもち、
家族に支えられながら、杖歩行で来院。
低心機能伴う大動脈弁閉鎖不全を基礎心疾患とした、慢性心不全の症例。
紹介医からは、在宅訪問診療の導入を検討して欲しいとのこと。
そこで患者さんから、このような言葉を聞きました。
「これまでどの先生も自宅で安静にしていなさいと言われ、訪問診療が良いと。
でも、わたしはまだこうやって歩けるし、たくさん運動もしたい。
クリニックに通院しにきたい。
その途中で転んだら、それはそれで人生。まだ通院でいいでしょう？」
70代のジャーナリスト男性Bさん、都内大学病院で心アミロイドーシスを
基礎心疾患とした難治性心不全で通院中。
右心不全の増悪により、下肢浮腫および下肢潰瘍からの感染を認める。
執筆活動も終盤にあり、自宅で命に代えてでもどうしても書き終えたい
と訴え、入院を拒んでいた。
当院紹介のうえ、在宅訪問診療の導入となる。
連日の補液、抗生剤投与、心不全治療にて、執筆活動を終える。
その後、一時的に炎症反応の改善を認めるが、
突然の悪寒・戦慄、炎症反応の再燃を認め、地域総合病院へ入院となる。
シームレスな医療
人生をつないでいるのは、患者さん本人である
医療者が患者のpoint, pointをつなげていくのではない
患者さんの地域の中での生活を理解し、
患者さんやその家族に寄り添いながらpoint, pointで支えて歩いていく

これからも私たちのクリニックでそんな医療を提供していきたいと思います。
Seamless medical care
I had a chance to speak about heart failure care in a conference which focused on "promoting community healthcare coordination - seeking for seamless medical care."
What is seamless medical care?
Is it about healthcare professionals creating an environment with compartmentalizing medicine into outpatient, in-patient and homecare? Or is it about healthcare professionals deciding a best suited environment for a patient?
As I questioned myself these questions, words from Dr. Lynne Warner Stevenson, Harvard University / Brigham And Women's Hospital came across my mind. She said...

Patients have their own past, present and future.
Everyone experiences birth, grow-up, become frail and death.
Along the way, we all feel happiness, anger, sadness and joy.
For patients, their lives are seamless connecting day to day base.
I believe the true seamless medical care is about staying on the patient's side to learn the path that they have taken and their goals in the future to support them realize their own lives with method of medicine.
Our clinic is doing our best to provide the seamless medical care for the patients.
90 years old male was introduced our clinic from a general hospital. He was walking on a cane with support of his family members. Etiology of his heart failure was aortic regurgitation with low ejection fraction. We considered providing home care for him. However, he told me "every physician I met recommended me home care, but I am healthy and able to move without a problem. I do not want home care and like to be an outpatient patient. If I fell on the way to the clinic, that would be just my life is not it? Let me pick"
A journalist of age 70 diagnosed with advanced heart failure due to cardiac amyloidosis as his primary disease visited my clinic. With exacerbation of heart failure, edema and infection from ulcer in his lower body were observed. He desired to stay home and finish up his writing and denied to be sent to hospital. He was introduced our clinic and started home care. He finished his writing while he was on hydration, antibiotic and heart failure treatment. Even though some improvement of his inflammation was observed, sudden chill and shivering occurred and inflammation came back causing him to be sent back to general hospital.
Seamless medical care...
Patients themselves are connecting their lives seamlessly and not healthcare professionals leading them one point to another. We should listen and understand the life of patients and be on their side whenever they need support.

Our clinic is willing to continue providing medical care with above philosophy within our mind.
